OUT OF OFFICE PROTOCOL

In order to mitigate community spread, we require all patients to answer these screening questions before they book. If a patient does not pass the screening, they will not be able to be seen for in-office care. Please call us directly for further guidance.

  1. Is the patient currently experiencing any of the following symptoms: fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea?

  2. In the past 2 weeks, has the patient been in contact with someone with known or suspected coronavirus (COVID-19)?

  3. In the past 2 weeks, has the patient been told by public health authorities or a healthcare professional to self-quarantine or isolate?

  4. In the past 2 weeks, have you traveled to one of the following states? https://www.chicago.gov/city/en/sites/covid-19/home/emergency-travel-order.html

In OFFICE PROTOCOL

Eyecare is essential. We're OPEN for all of your eye care needs. Operating with CDC guidance, practicing social distancing, we've added safety protocols to ensure the safety of our team, families & our clients.

- We reduced the number of appointments to reduce the number of patients in the office at one time.

- To minimize contact with other people in the building, please come alone to your appointment.

- Anyone entering and within the building MUST wear a mask or face covering.

- Please sanitize your hands upon arrival. Areas for handwashing will be available within our office.

- We will be thoroughly disinfecting patient contact areas before and after each patient with hospital-grade disinfectants.

- Whenever available and suitable to condition, patients will be seen via telehealth.

To expedite your visit and promote touch-free interaction, please text us a picture of your insurance card info and fill out our online medical history forms!

- We may extend glasses and contact lens prescriptions for those who don't wish to come in during the pandemic at the discretion of the doctors.

- Contact lens orders can be shipped with a $9 fee.

- Curbside pickup is available.
